ABSTRACT

OBJETIVO: Relatar a evolução de pacientes com artrose unicompartimental medial do joelho submetidos à artroplastia unicompartimental tipo Repicci II, com o propósito de avaliar a validade do procedimento. MÉTODOS: Participaram do estudo 36 pacientes com artrose unicompartimental do joelho, seis dos quais apresentavam acometimento bilateral, totalizando 42 joelhos. A idade variou de 54 a 82 anos, com média de 67 anos. Trinta e dois pacientes eram do sexo feminino e 10 do masculino. Os critérios clínicos de seleção foram: pacientes acima dos 50 anos de idade, nível de atividade leve/moderado, amplitude articular de pelo menos 90º de flexão e -10º de extensão, deformidade em varo menor ou igual a 5º, ausência de instabilidade ligamentar anterior e dor restrita ao compartimento medial. Os critérios radiográficos foram: artrose confinada ao compartimento medial com ausência ou mínima artrose patelofemoral. Utilizou-se a classificação de Ahlbãck para quantificar o grau de extensão da artrose, indicando-se cirurgia nos graus II, III e IV. Para acompanhamento dos resultados, utilizou-se a escala da Knee International Society. RESULTADOS: O tempo médio de seguimento foi de 36 meses. Não ocorreram casos de infecção pós-operatória, trombose venosa profunda ou embolia. Dois pacientes evoluíram com dor importante sem melhora com acompanhamento clínico, sendo necessária a revisão com prótese total do joelho. Não foram observados casos de soltura asséptica, afundamento do componente tibial ou desgaste precoce do polietileno. O escore médio pré-operatório foi de 45 para o joelho e de 57 para a função global do paciente. O escore pós-operatório foi de, respectivamente, 76 e 90 pontos. CONCLUSÃO: A artroplastia unicompartimental tipo Repicci II é uma boa opção no tratamento da artrose unicompartimental medial do joelho, trazendo resultados consistentes e implicando alto nível de satisfação dos pacientes, desde que técnica cirúrgica aprimorada, escolha...


OBJECTIVE:To report the evolution with medial unicompartmental arthrosis of the knee submitted to Repicci II unicompartmental arthroplasty in order to evaluate the validity of the procedure. METHODS: 36 patients with unicompartmental arthrosis of the knee were included in the study, six of them with bilateral involvement, in a total of 42 knees. Age ranged from 54 to 82, with a mean of 67 years. Thirty-two patients were female, and 10 male. Clinical section criteria were: patients had to be older than 50, with light/moderate level activity, joint amplitude of at least 90º of flexion and -10º of extension, varus deformity of less than or equal to 5º, no anterior ligament instability, and pain restricted to the medial compartment. Radiographic selection criteria were: arthrosis confined to the medial compartment, with none or minimal patellofemoral arthrosis. The Ahlbãck classification was used to quantify the degree of extension of the arthrosis, surgery being indicated for grades II, III, and IV. The Knee International Society scale was used to follow results. RESULTS: The mean follow-up period was 36 months. There were no cases of post-operative infection, deep venous thrombosis, or embolism. Two patients developed major pain with no improvement in the follow-up, and a revision with total knee prosthesis was required. No cases of aseptic loosening or early polyethylene wear were seen. The mean pre-operative score was 45 for the knee and 57 for the overall function of the patient. The post-operative score was 76 and 90, respectively. CONCLUSION: Repicci II unicompartmental arthroplasty is a good option of the treatment of medial unicompartmental arthrosis of the knee, and it achieves consistent results leading to high patient satisfaction provided that an improved surgical technique, the right choice of implants, and a strict selection of patients are observed.

ABSTRACT

OBJECTIVE: It is controversial whether age is associated with higher grade and worse outcome. Some studies have not found age to be related to outcome nor younger age to be associated with better response to therapy. MATERIALS AND METHODS: The study population consisted of 27 patients aged 55 years or younger and 173 patients 56 years or older submitted to radical prostatectomy. The variables studied were preoperative PSA, time to PSA progression following radical prostatectomy and pathologic findings in surgical specimens: Gleason score, Gleason predominant grade, positive surgical margins, tumor extent, extraprostatic extension (pT3a), and seminal vesicle invasion (pT3b). RESULTS: Comparing patients aged 55 years or younger and 56 years or older, there was no statistically significant difference for all variables studied: preoperative PSA (p = 0.4417), Gleason score (p = 0.3934), Gleason predominant grade (p = 0.2653), tumor extent (p = 0.1190), positive surgical margins (p = 0.8335), extraprostatic extension (p = 0.3447) and seminal vesicle invasion (p > 0.9999). During the study period, 44 patients (22 percent) developed PSA progression. No difference was found in the time to biochemical progression between men aged 55 years or younger and 56 years or older. CONCLUSION: Our findings suggest that age alone do not influence the biological aggressiveness of prostate cancer.

ABSTRACT

O objetivo do trabalho foi avaliar a prevalência de tecido glandular e neoplasia residual no retalho cutâneo de pacientes submetidas à mastectomia com preservação de pele (MPP). Quarenta e duas mulheres portadoras de carcinoma de mama, estadio 0 a III, foram submetidas à MPP entre junho de 2003 e janeiro de 2004. Antes da cirurgia, foram desenhadas duas linhas na pele da mama, representando as incisões da MPP e da mastectomia convencional. Após a cirurgia, o retalho cutâneo que permaneceria após a MPP foi retirado. Neste, foram avaliadas a presença de tecido glandular mamário, através da identificação de unidades ducto tubulares alveolares (UDTL), assim como, a presença de neoplasia residual, por microscopia óptica. Estes dados foram correlacionados com outros parâmetros clínicos e patológicos, através de teste exato de Fisher e curva ROC. Em 25/42 (59,5%) dos casos havia UDTL remanescente, associada significativamente com espessura do retalho cutâneo > 5 mm. Em 4/42 das pacientes havia neoplasia residual na pele, associada à espessura > 5mm e presença de UDTL. A curva ROC mostrou que conforme a espessura do retalho cutâneo diminui, também diminui a prevalência de UDTL. Foi concluído que alta prevalência de UDTL e de neoplasia residual estiveram associadas a espessuras de retalhos cutâneos > que 5 mm.


The objective of the study was to evaluate the prevalence of glandular tissue and residual tumor in the skin flap after skin-sparing mastectomies (SSM). Forty-two breast cancer female patients, stage 0 - III, without clinical skin involvement, underwent SSM from June 2003 to January 2004. Before surgery, two lines were drawn on the breast skin, representing SSM and conventional mastectomy incisions. After surgery, the skin flap that would remain after SSM was removed and immediate breast reconstruction was begun. The presence and amount of remaining glandular breast tissues were histologically evaluated in the skin flap, identifying terminal duct-lobular units (TDLU) under an optical microscope, as well as the presence of residual disease. These data were correlated with other clinical and pathological parameters, using the Fisher exact test and ROC curve. The prevalence of residual breast tissue in the sample was 59,5% (25/42) and the presence of TDLU was significantly associated with skin flaps thicker than 5mm. Residual disease was found in 9,5% (4/42) of the women and associated with skin flaps > 5mm thick and presence of TDLU. The ROC curve showed that, as skin flaps reduces in thickness, the presence of TDLU also decreases. It was concluded that a high prevalence of glandular breast tissue and residual disease in the skin flap were associated with skin flaps thickness > 5mm.

ABSTRACT

Foram estudados 48 pacientes de pediatria, atendidos na Unidade Básica de Saúde Cafezal durante o ano de 1993, com bronquite asmática. Os pacientes foram caracterizados com relaçäo a sexo, idade inicial do problema, conduta no momento da crise, fatores desencadeantes, antecedentes familiares asmáticos e nível de informaçöes a respeito do problema. Verificou-se que a proporçäo de 54,2 por cento das crianças eram do sexo masculino e 45,8 por cento do sexo feminino. O problema teve início antes do 1§ ano de vida em 60,4 por cento das crianças e até o 3§ ano de vida 79,2 por cento. A maioria dos pacientes é levada à Unidade Básica de Saúde (UBS) quando tem crise e muitos associam tratamentos caseiros. Entre os fatores desencadeantes da crise foram citados: mudanças de tempo (87,5 por cento), poeira (87,5 por cento), fatores emocionais (8,33 por cento) e outros (33,33 por cento), tais como lä, perfume, pêlo de animais, bebida gelada, etc. Foram identificados que 81 por centos dos pacientes têm algum antecedente familiar com bronquite asmática. Foi possível, também, verificar que a populaçäo näo possui informaçöes com relaçäo à bronquite asmática
